Remote Patient Advocate & Clinical Care Coordinator
Exclusively Remote
US-based healthcare client is seeking a Clinical Care Coordinator / Patient Advocate to support patients in a skilled care setting.
You will collaborate with nurses, physicians, PTs and OTs to understand patient needs and advocate for continued care when discharge may be premature.
The role requires strong clinical knowledge, empathy, and the ability to communicate complex medical information clearly in a remote, South Africa-based environment.
Full-time hours, PTO included.
